Step 1. Find a photographer who can take and print out passport sized photos. CVS and Walgreen's are two major stores that offer this service for low prices. Step 2. Collect your Birth Certificate or any proof of citizenship, your ID (this could be in the form of a Drivers License, your State ID, or Military ID), your passport photograph, and a blank check. Step 3. Go online to https://eforms.state.gov/Forms/ds11.PDF and fill and print out this form. Step 4. Take your documents you have collected and your photograph and go to your local passport acceptance facility. Most require appointments and they will be able to send over everything you need and have your passport to you as soon as they can based on what you paid for. 4.1. A passport acceptance facility can be a post office, a library, or your local courthouse. You can visit: https://iafdb.travel.state.gov/ to find your nearest facility.
